Statement of Purpose/Goals and Area of Research Interest
A one-two page paper should describe the applicant’s goals for doctoral education and for her/his profession or career. It should include a description of the applicant’s research area of interest and identify a potential faculty
member/s with whom the applicant might be able to develop a common research interest.

Writing Sample
A two-page position paper on a current social or health issue. (APA style)

Transcripts
Transcripts from each college or university attended are required, except from Georgia State University/Perimeter College. Transcripts are required regardless of length of stay or if the grades are listed on another school’s transcript. This also includes hospital schools of nursing. If you have attended Georgia State University/Perimeter College in an undergraduate status, all transcripts, except Georgia State University/Perimeter College, must be sent; these transcripts are not available for our use.

Licensure/Certification
All applicants (including international applicants) must hold a license in the United States as a registered nurse (RN license) prior to acceptance to the doctoral program. State and number should be listed on the resume.

Certificate of Immunization
This is required for accepted applicants who have never attended Georgia State University. If admitted, you must submit this form to Georgia State University Health Services prior to registration. The form can be printed from www.gsu.edu/health

Official TOEFL scores
(For applicants whose native language is not English). If available, please include a copy of your TOEFL score in your application packet. Scores cannot be more than 2 years old from the desired term of entry.
